Effective Monday, August 10, 2026, the Housing Questionnaire Form will be available in the Parent Portal.
The Housing Questionnaire Form allows families to provide current housing information online and helps schools identify students who may qualify for support and services under the McKinney-Vento Homeless Assistance Act.

Families can begin completing the Housing Questionnaire Form through the Parent Portal.

The Housing Questionnaire will be available in English and Spanish in the Parent Portal. Please contact Counseling and Student Support Services at [email protected] if the Housing Questionnaire is needed in other languages.

School staff will have access to review submitted responses and run reports as needed.

Question 19b - Temporary/Inadequate Residence Due to Financial Hardship, of student’s Demographics page can only be updated through the submission of a Housing Questionnaire. Staff can no longer edit this field directly for active or pre-registered students.

Timely review and processing of Housing Questionnaire responses help ensure eligible students receive appropriate McKinney-Vento services and support.
Schools may also enter Housing Questionnaire Form information in PowerSchool Admin when information is received via a paper form. Staff entering this information in PowerSchool Admin should refer to the Housing Questionnaire Form Job Aid for guidance.
